Application Procedures for Users
To enter a Radiation Controlled Area
The following procedures are required for those who enter a Radiation Controlled Area of New SUBARU.
These procedures are applicable to the users. · Radiation Worker registration (New SUBARU) → Those who conduct an activity (experiment)
· Application to enter a special area → Those who enter the specified place of the special area for a specified time
· Application for temporary entry to a Radiation Controlled Area → Those who enter for such a purpose as a tour of the facility
Required documents and procedures differ depending on the organization to which the person belongs. Those who are not users should refer to the following.

Registration of a Radiation Worker
Procedure required for all those who conduct an activity (experiment) in a Radiation Controlled Area of New SUBARU.

* Application for Radiation Worker registration for NEW SUBARU must be after (or at the same time as) the completion of registration for SPring-8.

Application form for Radiation Worker registration (New SUBARU)
As a general rule, it is necessary to submit the form 5-1NS, “Application form for Radiation Worker registration (New SUBARU) ”at least 10 days earlier. Those who have registered the year before also have to follow the same procedure. If there are any unclear items, please refer to FAQ or contact the front desk of the Safety Office.
Radiation Safety Training Date
It is necessary to attend the training regarding the handling of radioactive materials pursuant to Enforcement Regulation 21-2-1-4-a, b, and c of the Law Concerning Prevention of Radiation Injury by Radioactive Isotopes. State the date of attending such training which should not be longer than one year earlier. To state the planned date of the training is not acceptable.
It is not the date of attending the Safety Training for New SUBARU. We will verify the validity of the date by counting backward from the date of the registration (date of the planned visit as a reference). We may separately check before the planned use of the facility, if the validity period is about to expire or the date of application submission and the date of planned use are far apart.
The relevant training may be omitted if the person belongs to the organization, which is an authorized user, and possesses adequate knowledge and skills of the items of the relevant training. If the training is omitted, state the date of omission in the section for the date of training.
If training is not available at the organization, attend training offered at safety training facilities (refer to the last reference).
Date of an Ionizing Radiation Health Check
Let the legally-defined items of an ionizing radiation health check (refer to the following) be checked at a medical institution. The diagnosis by the physician should be verified at the organization to which the applicant belongs, before the date of visiting the physician (including the visit for a medical interview alone) is entered. Make sure that the date of visiting a physician is within 8 months (1 year for students) before the date of the planned visit to Japan Synchrotron Radiation Research Institute, regardless of the person who signed the document. The date of the planned visit to the physician is not acceptable. We will verify the validity of the date by counting backward from the date of the registration (date of the planned visit as a reference). We may separately check before the planned use of the facility if the validity period is about to expire or the date of application submission and the date of planned use are far apart.
(Reference) Ionizing radiation health check items
1) Medical interview: examination and evaluation of the presence or absence of a history of radiation exposure and subjective symptoms.
2) Blood test: hemoglobin content or hematocrit level, red blood cell count, white blood cell count, and differential white blood cell count.
3) Skin examination
4) Eye examination for cataract
Note) Other items than the medical interview may be omitted by the judgment of the physician. Check at your organization.

a. The registration of a user is valid until the end of the fiscal year (the end of March), except in case the form 5-2NS “Radiation Worker termination sheet (New SUBARU)” was submitted.

Radiation Worker Termination Sheet (New SUBARU)
When terminating the worker registration during a fiscal year due to changes in the organization, submit Form 5-2NS, “Radiation Worker termination sheet (New SUBARU)” as soon as possible.
If continued use of NEW SUBARU is planned after a change in the organization, submit a new application for Radiation Worker registration from a new department.
Radiation Worker Pregnancy Report (New SUBARU) for User
It is stipulated by law that the limit of the effective dose for pregnant female workers should be lower than that for female workers who are not pregnant. When a registered radiation worker of SPring-8 or New SUBARU has become pregnant, a “Radiation Worker Pregnancy Report” should be submitted to the Safety Office by the worker directly or via her department. The Safety Office will implement the relevant management up to one month after the due date that is stated in the “Radiation Worker Pregnancy Report.”
Application to Enter the Special Area New SUBARU
This procedure is necessary for those who enter the special area while the special area is set at New SUBARU.
When operation of the Accelerator is suspended for 7 days or longer, the regular Radiation Controlled Area may be designated as a “special area” and special management and operation may be implemented.
This application is not for users.
Application form to Temporarily Enter a Radiation Controlled Area - New SUBARU
This is a procedure for those who temporarily enter the controlled area for a tour of the facility of New SUBARU or any other work than radiation work.

An “Application form for tempory entry to Radiation Controlled Area” (writable using Adobe Reader etc.) must be submitted to the front desk of the Safety Office on the second floor of the Main Building or the New SUBARU office before 5 pm one day before the planned entry.
The downloaded document should be submitted in a duplex printed form.
‹Notes›
An individual dosimeter loaned out by the Safety Office (except for the special area) and the approved application form are necessary at the time of entry (Submitting only the application will not entitle you to enter the facility). After entry, enter the entry record on the back of the application form and submit it to the front desk of the Safety Office on the second floor of the Main Building with the individual dosimeter.
Consult the Safety Office on the second floor of the Ring Facility or New SUBARU office as soon as possible if it is planned to enter after office hours or on weekends and holidays.
Application is different depending on the applicant.
1) When the User is an observer, consult the New SUBARU office.
2) When personnel of the Laboratory of Advanced Science and Technology for Industry, University of Hyogo is the observer, the applicant can be the personnel of the said laboratory who is on the teaching staff of University of Hyogo.
Refer to the “FAQ/Frequently Asked Questions for Radioisotopes/Internationally Controlled Materials” and the procedures [English] when entering the form.

Users cannot apply for this procedure directly.
The documents regarding the use of the door of the carry-in entrance of New SUBARU (Application for use of the door for carry-in into the Radiation Controlled Area and Record of use of the door of the carry-in entrance) are found in the New SUBARU radiation monitoring room.
Work on specially-controlled radiation management
*This application is not for a Public or Contract Beam-line Users
What is specially-controlled radiation management work?
1. Work to need the presentation with documents of “ Radiation management work plan sheet”
- Work at the place that can cause it (a dose rate more than 6μSv/h) of the high dose of radioactivity
- Work to handle the article which I touch the management water (primary cooling water of the accelerator) directly or touched it
- Work to process the articles which there was in a machine room while accelerators※1 operates
- The work that processing (the through-hole opening to traffic, anchor casting) makes the article which there was during accelerator driving in the department in accelerator storing
2. Work to need the presentation with documents of “Radiation management work contact sheet”
- The work that work to affect the cover structure of the radiation generator
- Work to perform processing (anchor casting) on the storing outsider walls such as accelerators
- Work to process the wall of radiation controlled area border
3. Work to process the articles which there was in a machine room while accelerators
